Probably failed to put both locknuts on the wheelbearings, or screwed up the lock washer.


Could also be a complete failure of the wheel hub assembly.


I'd lean toward the wheelbearing being assembled incorrectly. Ususally we only see steer tires (single) dissappearing, the general rule is to not even look for the wheel once it comes off. More than likely you won't like what you find.
