Swallow and BJ should be the same button because that's a given (or it should be if you have one of the good ones). So that leaves you room for facial. Now swap positions with HJ and replace HJ with anal and your all set.

I think there is a cheat code to get anal, something like pressing the BJ button while holding the Vol +.
